(D) In the titration of a strong acid $(HCl)$ with a strong base $(NaOH)$,the initial $pH$ is very low due to the presence of $HCl$. As the base is added,the $pH$ increases slowly at first. Near the equivalence point,there is a sharp and sudden increase in $pH$ because the concentration of $H^+$ ions decreases rapidly. After the equivalence point,the $pH$ becomes almost constant as it is determined by the excess of $NaOH$ added.
