$-\sin y \frac{dy}{dx} = \cos (a+y) \cdot (1) + x \cdot (-\sin (a+y)) \frac{dy}{dx}$
Rearranging the terms to isolate $\frac{dy}{dx}$:
$x \sin (a+y) \frac{dy}{dx} - \sin y \frac{dy}{dx} = \cos (a+y)$
